Patient’s who have progressive life limiting illnesses and are experiencing symptoms such as pain, shortness of breath, nausea, or fatigue can benefit from palliative care services. Individuals who are still undergoing active curative treatment, those who may not be ready for hospice care, or choose not to select hospice care are also eligible for palliative care.
Palliative Care Consultation
Consultations are provided by the palliative care team in a number of healthcare settings including:
- Extended Care Facilities
- Assisted Living Facilities
- Acute Care Facilities
- Out-Patient Clinics/Offices
- Private Homes
Physicians, physicians assistants and nurse practitioners can make a referral to the Palliative Care program.
How is Palliative Care Paid For?
